What is a tourniquet, how to make one, and what types exist?

In emergency situations, a tourniquet can mean the difference between life and death. Knowing what it is, how to use it correctly, and when to apply it is essential for controlling severe bleeding. In this article, we explain everything you need to know about tourniquets and show you the best options available atSERMILITAR.

¿Qué es un torniquete?

A tourniquet is a device used to stop blood flow in a limb when there is severe bleeding that cannot be controlled with direct pressure. Its function is to compress the blood vessels to prevent excessive blood loss, which can be vital in emergency situations such as accidents, gunshot wounds, or explosions.

Types of turnstiles

There are different types of turnstiles, each designed for specific situations and with various methods of application. The most commonly used are:

1. CAT Tourniquet (Combat Application Tourniquet)

It is the most popular among military personnel and emergency teams. It fastens with a strap and a pressure rod for quick and effective hemorrhage control. The CAT tourniquet was developed in the early 2000s and patented in 2003 by U.S. military doctor Dr. Richard Schwartz and combat veteran Bryan J. Fry, with the goal of providing a more effective and easy-to-apply method for controlling bleeding on the battlefield.

The U.S. Army officially adopted the C-A-T in 2005, and since then it has become the standard for hemorrhage control in tactical and emergency settings, being used by military forces, emergency teams, and police worldwide.

2. SOFTT-W Tourniquet

Similar to the CAT, but with a wider band and a more robust closure system, ideal for use in extreme conditions. This was developed by the American company Tactical Medical Solutions (TacMed Solutions); incorporating a wider strap helps reduce tissue damage and improve effectiveness in hemorrhage control.

The SOFTT-W has been adopted by various military units and security forces, standing out for its high durability, quick-release aluminum alloy buckle, and ease of use with one hand.

3. SICH Tourniquet

Developed in the heat of the Ukraine war following the Russian invasion of its territory, the SICH tourniquet is, so to speak, an evolution of the CAT, featuring a lever made of more durable aluminum and an adjustment system that allows us to secure the tourniquet comfortably and safely with one hand.

This tourniquet has been well used on the battlefield and is an ideal option for military personnel and law enforcement agencies.

4. Rubber tourniquet (or improvised)

Although it is not recommended in most cases, in extreme emergency situations elastic bands or clothing can be used to improvise a temporary tourniquet. Some brands, such as Pentagon, have belts like the Hemantas which, being elastic, allow us to use them as an emergency tourniquet in extreme situations.

At SERMILITAR you can find different models of tourniquets and accessories such as tourniquet pouches for safe transport.

How to Make and Apply a Tourniquet Correctly?

The improper use of a tourniquet can cause damage to tissues and nerves, so it is crucial to follow the correct steps:

  1. Identify the hemorrhage:If the bleeding is heavy and cannot be controlled with direct pressure, a tourniquet is needed.

  2. Place the tourniquet:Apply it about 5-7 cm above the wound, avoiding placing it over a joint.

  3. Adjust and tighten:Make sure it is tightened properly. If the bleeding does not stop, tighten it further.

  4. Secure the tourniquet:Use the turnstile locking system to prevent it from loosening.

  5. Register the time: Note the time the tourniquet was applied. It should not remain in place for more than 2 hours without medical attention.

¿Cuándo usar un torniquete?

A tourniquet should only be used in cases of severe bleeding in the arms or legs when other bleeding control methods have failed. Some situations where it is necessary include:

  • Wounds from stabbing or gunshot.

  • Traffic accidents with partial or total amputations.

  • Explosions or injuries from heavy machinery.
